COVID-19 Vaccines & Testing

Walk-ins also welcome! We have rapid Covid-19 testing available! Results in 15 minutes. 

Vaccines are available on Saturdays and Sundays. The clinic will be giving the Moderna Covid 19 vaccine. Now available for anyone 18 years and up.  Walk-ins welcome! Please print off and complete the Covid 19 Vaccine Screening and Consent Form. Also, please bring a copy of your Florida's drivers license or proof of Florida residency along with a copy of your health insurance card (if you have insurance). Free Covid care for anyone, even with no insurance.
